To register a student, please print and complete ALL documents, including the Health Form and Home Language Questionnaire. They are part of the registration packet. Please be sure to read carefully, and complete and sign all forms where required:

Universal Pre-Kindergarten & Targeted Pre-Kindergarten
If you are registering your child for Universal Pre-Kindergarten (UPK) or Targeted Pre-Kindergarten (TPK), please print and complete the following forms and return them to Nikki DeDominicis, Registrar, in person or by mail (215 Harry Howard Avenue, Hudson NY 12534):

Important vaccine information
